OutFront JB Family Residential Care LLC. is very proud to be able to provide Home and Community-Based Services Supports to Persons with Intellectual and Developmental Disabilities. What is Community Living Supports? OutFront, believes Community Living Supports are supports to individuals with disabilities, like individuals without disabilities, should have choice and self-determination in housing and in the health care and related support services they receive that should empower all Individuals to recognize and utilize their community more independently.
As we provide supports to Persons with Disabilities, our goal is to provide staff that can be teachers. As we assist those we support, we believe that they must do, and learn to do as much as they possibly can themselves. Our staff will assist them and teach them as they go through their daily lives and activities. If a person we support does not know how to dress or may have a physical challenge that makes it difficult, our staff will find ways to support them so they can learn how to dress appropriately. This is the same process for any of their everyday living activities.
Living in the Home and Community-Based Services and taking care of everyday living activities in our homes is not all there is to living in the community. Being an active member in the community also involves being active as a neighbor, a consumer, taking part in community activities, utilizing stores, public transportation, and services, finding friends and groups to participate in, and giving back by volunteering and serving in the community.
OutFront assists those we support in being active in the community by developing specific and individualized programs which allow them to be their own individual. All of us have similarities and differences. We all define ourselves based on what it is we like to do, our goals, wants, desires, and needs. Once we have explored these areas with the person, we then assist them in developing strategies and systems to realize these outcomes.
OutFront also recognizes the importance of appropriate, organized health and medical supports for people to live successfully in the community. We assist those we support by empowering them to pick their doctors and health care supports. We also assist them in any way they may need to assure these supports are in place and meet their needs.
OutFront 's goal is to teach those we support independence in all areas of their life, to teach them about their community, to assist them in utilizing their community and the support structures which are there for them as well as for all of us. In turn it will assist all people in the community to better understand each other and how we can all positively enhance each other’s lives.

The relationship between the staff and the family is critical to the success of the Program. This relationship is a partnership based on trust, respect, and open communication. The staff's role is to empower families to identify their needs and assist families to make key support decisions for their individual and family and obtain the support they require.
Caregivers Description:
OutFront staff/caregivers are recognized as a prominent part of our group home care and supportive services programs. Personal care consists of client safety, maintenance and support primarily involving a combination of personal assistance and homemaker activities. As with other services, these activities are performed under the direction of the care plan is to be followed exactly.
Caregiver’s responsibilities include:

Care Plan
You are to provide the care as it is outlined in the care plan. If the client or family requests care that is not on the care plan, or the care needed is not on the care plan, please contact the RN or the operations manager immediately and report the need for changes. Do not provide care that is not on the care plan without the OutFront JB Family approval.
Administration/Dispensing of Medication and Non-Prescription
Medication
OutFront JB Family employees can only dispense medication out of bubble packs that come directly from the pharmacy. The bubble pack will have the residents name, day/time medication should be taken, and it should include the MAR to document time and date taken. No other methods are tolerated under any circumstance.
We will not administer non-prescription directly to a client. Employees can only assist. Assisting is defined as bringing the prescription (bottle or container), opening the prescription, and allowing the client to take the prescription out of a cup, from their hand or the container and take the prescription themselves. Direct care staff is not allowed to put medication directly into the resident’s mouth.
