Tailored Person-Centered Plan for Members

At OutFront JB Family Residential Care, LLC, we take pride in offering developmental programs that are tailored to meet the unique needs of our members. Our programs are focused on assisting each member to reach his or her maximum physical, social, emotional, and intellectual potential. We have a 24/7 staffed facility that is safe for daily living and provides a family-like environment that enhances each member’s pursuit of independence. All of our members have a person-centered plan (PCP) directed by various desires and needs identified by them and other important people in their lives. Our members work on social and community integration and develop and maintain relationships with their family and friends.

Promoting Sustainable Independent Living

Home and Community-Based Services-Living and Inclusion:

OutFront JB Family Residential Care LLC. is very proud to be able to provide Home and Community-Based Services Supports to Persons with Intellectual and Developmental Disabilities. What is Community Living Supports? OutFront, believes Community Living Supports are supports to individuals with disabilities, like individuals without disabilities, should have choice and self-determination in housing and in the health care and related support services they receive that should empower all Individuals to recognize and utilize their community more independently.

As we provide supports to Persons with Disabilities, our goal is to provide staff that can be teachers. As we assist those we support, we believe that they must do, and learn to do as much as they possibly can themselves. Our staff will assist them and teach them as they go through their daily lives and activities. If a person we support does not know how to dress or may have a physical challenge that makes it difficult, our staff will find ways to support them so they can learn how to dress appropriately. This is the same process for any of their everyday living activities.

Living in the Home and Community-Based Services and taking care of everyday living activities in our homes is not all there is to living in the community. Being an active member in the community also involves being active as a neighbor, a consumer, taking part in community activities, utilizing stores, public transportation, and services, finding friends and groups to participate in, and giving back by volunteering and serving in the community.

OutFront assists those we support in being active in the community by developing specific and individualized programs which allow them to be their own individual. All of us have similarities and differences. We all define ourselves based on what it is we like to do, our goals, wants, desires, and needs. Once we have explored these areas with the person, we then assist them in developing strategies and systems to realize these outcomes.

OutFront also recognizes the importance of appropriate, organized health and medical supports for people to live successfully in the community. We assist those we support by empowering them to pick their doctors and health care supports. We also assist them in any way they may need to assure these supports are in place and meet their needs.

OutFront 's goal is to teach those we support independence in all areas of their life, to teach them about their community, to assist them in utilizing their community and the support structures which are there for them as well as for all of us. In turn it will assist all people in the community to better understand each other and how we can all positively enhance each other’s lives.

OutFront Principles of the Family Support:

The Family Support is based on family centered practice and uses a holistic approach to supporting families:

The relationship between the staff and the family is critical to the success of the Program. This relationship is a partnership based on trust, respect, and open communication. The staff's role is to empower families to identify their needs and assist families to make key support decisions for their individual and family and obtain the support they require.

  • respecting individual families' lifestyles, values and stated priorities in caring for their individual with a disability.
  • identifying and recognizing families' strengths and parenting expertise
  • taking account of other factors not directly related to the individual's disability which impact upon family functioning.
  • considering and addressing the needs of other family members when necessary
  • assisting families to identify and build a range of informal supports around the whole family.
  • being responsive to family crises or immediate needs
  • assisting families to plan and develop individualized, flexible, and responsive supports that can change over time as the families' needs change.

OutFront Principles Home and Community-Based Services, Living and Inclusion

  • OutFront Individuals should have the opportunity to live like people without disabilities. They should have the opportunity to be employed, have a place to call home, and be engaged in the community with family and friends.
  • OutFront Individuals should have control over their own day, including which job or educational or leisure activities they pursue.

Choice:

  • OutFront Individuals should have the opportunity to make informed choices. They must have full and accurate information about their options, including what services and financial support are available in integrated settings.   They should have the opportunity to visit integrated settings and talk to individuals with similar disabilities working and living in integrated settings. Their concerns about integrated settings should be explored and addressed.
  • OutFront should have control over where and how they live, including the opportunity to live in their own apartment or home. Living situations that require conformity to a collective schedule or that restrict personal activities limit the right to choose.

 Employment:

  • OutFront Individuals should have the opportunity to be employed in non-segregated, regular workplaces. Virtually all individuals can be employed and earn the same wages as people without disabilities. When needed for such employment, they should have access to supported or customized employment. They should be afforded options other than sheltered work, day treatment, clubhouses, and other segregated programs.

Housing:

  • Virtually all individuals we support can live in their own home with supports. Like people without disabilities, they should get to decide where they live, with whom they live, when and what they eat, who visits and when, etc.
  • To this end, individuals with disabilities should have access to housing other than group homes, other congregate arrangements, and multi-unit buildings or complexes that are primarily for people with disabilities. They should have access to “scattered site” housing, with ownership or control of a lease. Housing should not be conditioned on compliance with treatment or with a service plan.
